Clackamas County intends to amend its 2012-2016 Consolidated Plan in the following manner:
Program Year 2015-2016 (2015 Action Plan)
Rex-Putnam School Based Health Center 2015 – Clackamas County Community Development will use $100,000 of CDBG reprogrammed funds for this new project. The project will include renovation of 3134 square feet of interior space within Rex-Putnam High School to create a school based health center facility with three exam rooms, a lab and pharmacy dispensary, a rest room and several counseling offices. Construction will be completed by the end of the summer 2017, and Rex-Putnam Health and Wellness Center will begin serving students in September of 2017. The Project will be completed in partnership with the North Clackamas School District and has an estimated total project cost of $900,000. The $100,000 of CDBG reprogrammed funds allocated for this new project will have no impact on any project currently planned and budgeted in the 2015 program year.
