PUBLICATION SUMMONS TO:
DUSTIN HOWARD SURRATT
IN THE NAME OF THE STATE OF OREGON, you are hereby required to appear and answer the Petition filed in the above-entitled court on or before the expiration of four (4) weeks from the date of the first publication of this Summons; if you fail so to appear and answer, Petitioners, for want thereof, will apply to the above-entitled court for the relief prayed for in its Petition, to wit:
(a) Termination of your parental rights as to the minor child Rowan Rose Surratt;
(b) The adoption of the minor child by Daniel Joseph Murphy and Molly Ellen McGivem Murphy, so that Rowan Rose Surratt is the child of petitioners for all legal intents and purpose;
(c) Changing Rowan Rose Surratt’s name to Rowan Rose Murphy;
(d) Ordering the Vital Statistics Unit to issue a new birth certificate for the child; and
(e) Sealing all documents in this matter.
The Court has issued an Order to Appear and Show Cause why this relief should not be granted, if you fail to appear within 30 days of the first date of publication of this notice, the court will enter the requested judgment of adoption without your consent.

This Summons is published by order of Circuit Judge Kristan Thompson the above entitled court made and entered on the July 20, 2016 directing publication of this Summons once each week for four (4) consecutive weeks in a newspaper published and of general circulation in Washington County, Oregon.
