PUBLIC NOTICE CITY OF MADRAS, OREGON REQUEST FOR PROPOSAL INSURANCE AGENT OF RECORD The City of Madras is soliciting Requests for Proposals for Insurance Agent of Record services for the City’s risk management program; property, casualty, liability, and auto insurance and for employee insurance and related benefit programs; workers’ compensation, medical including vision and prescription coverage, dental, long term disability, life, AD & D, and flexible spending accounts and other risk management services. The contract for audit services will include a three-year term commencing with the fiscal year beginning July 01, 2020. All proposals must be hand delivered, by calling to schedule an appointment for delivery, to the City of Madras by no later than May 14, 2020 by 5:00 p.m. Proposals must be clearly marked City of Madras Insurance Agent of Record Proposal and directed to: City of Madras Attn: Charo Miller, HR & Admin.
