NOTICE OF CORPORATE DISSOLUTION Gresham Women’s HealthCare, P.C., was dissolved effective September 1, 2020. This notice is published, pursuant to ORS 60.644, to notify any creditors of Gresham Women’s HealthCare, P.C., of the dissolution and provide a final opportunity to make claims against the corporation. To file a claim, a creditor must send a letter describing the claim to the address listed below. The letter must contain all of the following: (1) A detailed written description of the products or services and the dates they were provided to Gresham Women’s HealthCare, P.C. A copy of the relevant itemized invoice would fulfill this requirement; (2) A copy of any correspondence, including billing statements, between the creditor and Gresham Women’s HealthCare, P.C., related to the claim; and (3) The name, address, and telephone number of a creditor contact regarding the claim. Claims must be sent to B. Edward Yanke, D.O., 2150 NE Division, Suite 202, Gresham, OR 97030.
