Separate sealed bids for the LAKEVIEW HEAD START PROJECT located in Lakeview, Oregon, will be received by Ray Simms, Town Manager, at 525 North 1st Street, Lakeview, Oregon, 97630, until 1:00 P.M., local time, May 19, 2016. The bids will be publicly opened, announced, and made available for public inspection at 1:00 PM on May 19, 2016. The First-Tier Subcontractor Disclosure Form, which is submitted separately, is required at 4:00 PM on May 19, 2016. The bids shall be mailed or delivered; fax transmissions or e-mails will not be accepted.
Generally the work consists of construction of an approximately 6,481 square foot building for the use as a Head Start facility, site work, and associated improvements.

Bidders must be pre-qualified. Bidders must submit by mail or deliver the Contractor’s Pre-Qualification Form which is included in the contract documents, or the State of Oregon Department of Administrative Services Pre-Qualification Application Form on or before the Mandatory Pre-Bid Meeting of May 4, 2016 at 1:00 PM. Fax or e-mail transmissions of Pre-Qualification Forms will be accepted only if the Contractor confirms receipt.
All of the entire Bidding Documents must be submitted or the bidder will be considered nonresponsive.
This is a public works contract and no bid will be received or considered by the public contracting agency unless the bid contains a statement by the bidder, as part of their/his/her bid that the provisions of ORS 279.350 (Prevailing Wage Rates) are to be complied with. A copy of the Prevailing Wage Rate Statement is included in the contract documents. The Federal Labor Standards Provisions are also included in the Contract, and the Contractor is required to pay the higher of the two wage rates.
No bids will be considered unless fully completed in the manner provided upon the bid schedule provided by the owner and accompanied by the bid bond or a certified check executed in favor of the owner in an amount not less than 5% of the total amount of the bid. Bid bond or check will be forfeited should the selected bidder neglect or refuse to enter into contract and provide suitable bond for faithful performance of the work.
The successful bidder shall provide a performance bond, as hereinafter stipulated, for the full amount of the contract within 10 days after awarding of the contract, i.e. contract date. No bidder may withdraw or modify their bid after the hour set for the opening thereof until after the lapse of thirty (30) days from the bid opening. The owner reserves the right to reject any or all bids not in compliance with all prescribed bidding procedures and requirements and other applicable laws, and the owner may reject for good cause any or all bids upon the owner finding that it is in the public interest to do so. Owner may waive any informality in the bidding process.
There will be a MANDATORY Pre-Bid Meeting May 4, 2016 at the Town Hall Annex, 525 North 1st Street, Lakeview, Oregon 97630, at 1:00 PM.
Bidders must be registered with the Oregon Construction Contractors Board.
The preparation of the contract documents, the project drawings, and the construction work under this contract is funded in its entirety with federal grant funds from the Oregon Community Development Block Grant Program.
