BASIC TELEPHONE, BROADBAND, AND LIFELINE SERVICES

PUBLIC NOTICE MONITOR COOPERATIVE TELEPHONE COMPANY DBA MONITOR TELECOM BASIC TELEPHONE, BROADBAND, AND LIFELINE SERVICES Monitor Cooperative Telephone Company DBA Monitor Telecom is a telecommunications provider who provides basic and enhanced services within its service territory. Monitor Telecom is an Eligible Telecommunications Carrier and as such, receives support from the Federal Universal Service Fund. Local service charges are as follows: Single Party Residential Basic Service – $17.60/month; Single and Multi-Party Business Basic Service – $19.20/month; State Mandated Emergency 911 Services for each telephone number – $1.00/month; Federal Mandated Subscriber Line Charge for Single Residential Line – $6.50/month; Single Business Line – $6.50/month; Multi-Business Line – $9.20/month. Effective January 1, 2021, the Oregon E911 tax rate will increase from $1.00 to $1.25 per month for each subscriber line or device capable of reaching 9-1-1. Customers of basic service have access to the public switched network, minutes of use for local service provided at no additional charge and access to emergency 911 services. Toll limitation services are also available for qualifying low-income customers. Lifeline service is available for qualifying low-income customers. The $9.25 federal Lifeline benefit may be applied to qualifying broadband service. Effective December 1, 2020, the federal Lifeline benefit for qualifying voice service will decrease from $7.25 to $5.25. The $3.50 state Lifeline benefit may be applied to qualifying service.
