Pursuant to ORS 60.644, notice is hereby given that Decision Impact, Inc., an Oregon domestic business corporation, whose last known principal office was located at 11856 SW Palermo Street, Wilsonville, Oregon 97070, is dissolved effective December 31, 2020. All persons having claims against Decision Impact, Inc. are required to present the claim in writing, including the name, address, and telephone number of the claimant(s), a description of claim alleged, and supporting documentation, to Donald D.
