The Trust was established under agreement dated April 18, 1996 and was amended in its entirety on March 12, 2019. First Republic Trust Company, Janice L. K. Hoffman, Karine C. H. Baer, and Brennan K. Hoffman are the Co-Trustees of the Trust. All persons having claims against the Trust are required to present them in writing to the undersigned Co-Trustee in care of attorney Jonathan J.
