The proceeding is regarding the above-named child, Kali Ann Jensen, who was born on July 23, 2018, in Reno, Washoe County, State of Nevada. YOU ARE HEREBY INFORMED THAT YOU HAVE THIRTY (30) DAYS AFTER THE DAY ON WHICH YOU WERE SERVED WITH THIS NOTICE TO FILE A MOTION AND MEMORANDUM TO INTERVENE REQUIRED BY UTAH CODE ANNOTATED 78B-6-110(6)(a) IF YOU INTEND TO INTERVENE IN OR CONTEST THE ADOPTION. FAILURE TO RESPOND WILL RESULT IN, WITHOUT FURTHER NOTICE TO YOU, WAIVER, FORFEITURE, AND TERMINATION OF YOUR PARENTAL RIGHTS TO THIS MINOR CHILD. Pursuant to Utah law, because you have been served with this Notice of Adoption proceedings, if you wish to contest the adoption you shall file a motion to intervene in the adoption proceeding within 30 days after the day on which you were served with this Notice of Adoption Proceedings, and that motion shall set forth specific relief sought and be accompanied by a memorandum specifying the factual and legal grounds upon which the motion is based. Any response to this notice and the proposed adoption must be filed with the Third Judicial District Court, Salt Lake County, 450 South State Street, Salt Lake City, UT 84111, in Case No. 183901920, and a copy of the response shall be also mailed to Derek J.
